Osteonecrose da cabeça femoral por uso de corticosteroides no tratamento da COVID-19: relato de caso

Resumo

A osteonecrose não traumática da cabeça femoral (ONCF) é caracterizada pela morte celular dos osteócitos e alterações nos osteoblastos e osteoclastos, frequentemente causada pelo uso prolongado de corticosteroides. Outros fatores de risco incluem doenças autoimunes, álcool, radioterapia e infecções. O presente relato descreve o caso de um homem de 55 anos que desenvolveu ONCF após tratamento com corticosteroides para COVID-19. A ONCF pode ser insidiosa, manifestando-se com dor e limitação de movimento, sendo a ressonância magnética (RM) o método diagnóstico mais eficaz. O tratamento varia de conservador a cirúrgico, dependendo do estágio da doença. O diagnóstico precoce e o acompanhamento multidisciplinar são essenciais para um melhor prognóstico. Pesquisas futuras devem focar em desenvolver diretrizes de tratamento da COVID-19 que reduzam a incidência de ONCF, explorando alternativas terapêuticas seguras e eficazes.
