Telangiectasia hereditária familiar: uma causa rara de trombose de porta

Resumo

Telangiectasia hemorrágica hereditária (THH) é uma doença autossômica dominante que cursa com malformações arteriovenosas que afetam pele, pulmões, cérebro, superfícies mucosas e trato gastrointestinal. A doença se apresenta de diversas formas clínicas como epistaxe, sangramento gastrointestinal e sintomas decorrentes das malformações arteriovenosas cerebrais e pulmonares. A THH confere risco aumentado para tromboembolismo venoso. Apresentamos uma mulher de 57 anos com THH que apresentou epistaxe de repetição, shunt arteriovenoso pulmonar e trombose de porta com transformação cavernomatosa. Ela foi submetida a cauterização nasal, embolização pulmonar, e anticoagulação. A paciente recebeu alta hospitalar com remissão dos sintomas e rivaroxabana oral.
