Interconnections between Endometriosis, Infertility, and Sleep: a systematic review

Introduction: Endometriosis is a chronic gynecological condition that affects the physical and mental health of women of reproductive age, associated with chronic pain, sexual dysfunction, and infertility. Sleep disorders are common in this population, influenced by hormonal fluctuations and the interaction between the circadian and reproductive systems. These alterations compromise quality of life and reproductive function, although their relationship with infertility remains underexplored. This systematic review investigates the interrelation between endometriosis, sleep, and infertility, highlighting the importance of integrated therapeutic approaches. Materials and Methods: A systematic search was conducted in the LILACS, PubMed, and SciELO databases using the descriptors “Endometriosis,” “Infertility,” and “Sleep.” Only peer-reviewed studies published in the last five years, freely accessible, and in English were included. Studies with small samples, not peer-reviewed, or in other languages were excluded. Methodological quality was assessed using the Cochrane RoB 2.0 and Newcastle-Ottawa Scale tools, while publication bias was analyzed using Egger’s test. Statistical analyses were performed using SPSS software, adopting p < 0.05 as the significance level. Results: The reviewed studies indicate that endometriosis negatively impacts sexual function and sleep quality, aggravated by pelvic pain, anxiety, and depression. Women with the condition have a higher prevalence of sleep disorders and more intense symptoms of mental disorders. Findings reinforce the need for multidisciplinary interventions that address both physical and psychological aspects. Conclusion: Sleep plays a fundamental role in the quality of life, mental health, and reproductive function of women with endometriosis. Integrated management of pain, sleep disorders, and mental health is essential to improve reproductive outcomes. Future studies should further investigate this relationship.
