Stiff Left Atrial Syndrome is an important and incomplete complication (1.4% to 8%) after ablation, it is related to the extensive of the scar in the left atrium (LA), which may induce a reduction in LA function and pulmonary hypertension. The objective of this study is to provide subsidies for the diagnosis of this pathology, besides reviewing the literature, updating the diagnostic and therapeutic methods. We report the case of a female patient with previous history of atrial flutter who underwent ablation, evolving with SAER. Pre-ablation risk factors: diabetes; small fright; obstructive sleep apnea and non-AE scarring. The specialist sustains in diagnostic clinical, modification of pressure of the pressure of pulmonary artery. Limitations to diagnosis are symptoms that may be present in some patients, making clinical differentiation or pressure assessment difficult. Therapeutic options are diuretics and Sildenafil in case of partial response to diuretics. The success of post-ablation diagnosis and the taking of an alert from the correct onset are the presence of a clinical sign, adapting to the patient so that it can progress.
