Endoscopic diagnosis of primary aortoenteric fistula: case report

Study type: Case report. Importance of the problem: Primary aortoenteric fistula has an incidence of 0.04% to 0.07% in the population, with mortality of almost 100% if left untreated or when associated with gastrointestinal hemorrhage. The diagnosis made by upper digestive endoscopy represents only 25% of the cases, with computed tomography as the gold standard to confirm this pathology. Comments: In this study, we report a rare condition found in the third part of the duodenum, with intense active bleeding, being diagnosed by upper digestive endoscopy and performed aneurysm hemostasis with endoscopic “clip” to control bleeding. Then the patient stabilized from the hemodynamic point of view, however, due to the severity of the case, he was referred to the Intensive Care Center (ICC) for the purpose of better care and computed tomography the next day, and possible surgical intervention. However, the patient evolved with hypertensive symptoms, with rebleeding (11 hours after diagnosis), without success with clinical treatment and without condition to perform new endoscopic hemostasis, evolving to hypovolemic shock, followed by cardiorespiratory arrest and death.
