Challenges in diagnosis of atrioventricular nodal reentry tachycardia: a bibliographic review

The atrioventricular nodal reentry tachycardia (AVNRT) is characterized by the re-entry of the electrical impulse in the atrioventricular node (AVN) area. Usually, it comes forward in a benign way, being twice as frequent in women than in men. The diagnosis is based on the patient’s clinical presentation and the 12-lead ECG and is characterized by pseudo r ‘on lead V1 or pseudo deflection S’ on lower leads. Possible difficulties in the diagnosis can be related to the time elapsed from the patient’s trip to the hospital, the short duration of the crises and the interval between them and the absence of previous cardiovascular diseases. This review aims to analyze the diagnostic strategies of AVNRT, based on scientific publications in the following databases: National Library of Medicine (PubMed), Scientific Eletronic Library On-line (SciELO) and US National Library of Medicine (NCBI). Due to the wide and nonspecific symptoms and the occurrence of spaced and short-lived crises, AVNRT can be mistakenly diagnosed as an anxious condition. It was observed that the 24-hour Holter is the most used exam in clinical practice, but one of the least effective. Despite the high cost and restricted access, diagnostic tests such as the external looper, the EEF and even the 48-hour Holter have significantly higher detection rates, making the diagnosis of AVNRT easier.
