Chronic ulcerative stomatitis: a case report and diagnostic review

Chronic ulcerative stomatitis (CUS) is an autoimmune disease that is still poorly understood and possibly underreported due to the enormous clinical and histological similarity with other conditions. Thus, several cases of Oral Lichen Planus (OLP), for example, may actually be manifestations of CUS. Other diseases, such as Pemphigus Vulgaris (PV) and Systemic Lupus Erythematosus (SLE), should be part of the differential diagnosis. To distinguish between CUS and other oral manifestations, the gold standard is Direct Immunofluorescence (DIF). As a result, there is a granular profile of immunoglobulins against nuclear proteins of keratinocytes, located in the basal and parabasal layers of the epithelium. Hence, it presented the clinical and immunopathological findings of a 35-year-old woman, complaining of desquamative gingivitis, erythema and pain. Initially, a diagnosis of OLP was obtained, and after drug therapy and DIF were unsuccessful, there was a conclusive diagnosis of CUS. From this case, we seek to discuss the various clinical, histological and immunological aspects of CUS, given its relevance in dental practice.
