It is known that, in addition to incarceration, release also has lasting effects on the health of people who have been deprived of liberty, sometimes meaning new care opportunities, in others representing a serious risk to returning to social life. This paper aimed to map existing Brazilian initiatives for the health care of people discharged from the prison system, with shared management equipment to support this care. This is a documentary, descriptive, and crosssectional research, with a qualitative approach, whose secondary data were collected from the institutional websites of the States of the Federation from June to July 2021. The updated during the editorial review process of the article. The data were analyzed considering the forwarding to the health network from organs for the care of those discharged from prison. It was found that most of initiatives to assist the discharged population in a format other than the social offices do not mention health referrals on their official websites. Given that most agreements with the Programa Fazendo Justiça are recent, exploratory research is needed to describe how referrals to the Brazilian National Public Health System network in social offices have been conducted, thereby serving as a mechanism for policy strengthening and evaluation.
