Resultados funcionais de longo prazo após artroplastia total de quadril: estudo transversal com grupo controle

Resumo

A artroplastia total de quadril (ATQ) é eficaz para redução da dor e restauração funcional, porém déficits de força e alterações de marcha podem permanecer mesmo após a recuperação clínica. Objetivo: Comparar força muscular do quadril, funcionalidade, marcha e qualidade de vida entre pessoas submetidas à ATQ e indivíduos controle sem cirurgia. Método: Estudo transversal. Foram avaliados pacientes submetidos à ATQ pela técnica de Moore (≥ 12 meses de pós-operatório), comparados a um grupo controle pareado por idade e sexo. A avaliação incluiu: força muscular isométrica (dinamômetro manual), funcionalidade (Harris Hip Score – HHS), qualidade de vida (WHOQOL-bref) e análise cinemática da marcha em dois planos. Testes t e Mann-Whitney foram utilizados para comparação intergrupos (p ≤ 0,05). Resultados: Quarenta e cinco participantes foram incluídos (ATQ= 25; controle= 20). O grupo ATQ apresentou menor força muscular em todos os grupos avaliados (p ≤ 0,001) e menor amplitude de movimento de quadril e tronco durante a marcha, quando comparado ao controle. A função pelo HHS foi classificada como razoável. Não houve diferença estatisticamente significativa na qualidade de vida entre os grupos. Conclusão: Pacientes submetidos à ATQ apresentam menor força muscular e alterações cinemáticas da marcha quando comparados a indivíduos sem cirurgia, com funcionalidade considerada razoável e níveis satisfatórios de qualidade de vida no momento da avaliação.
