Physiotherapeutic approach to the patient with eosinophilic granulomatosis with polyangiitis: a case report

Objective: This study describes the case of a patient with tetraparesis due to Eosinophilic Granulomatosis with Polyangiitis (EGPA) and aims to demonstrate the results of physiotherapy care for a patient with EGPA during the period of hospitalization in a specialized rehabilitation center, as well as procedures used during this process. Method: The evaluation was performed according to the evaluation form of the specialized center with inspection, analysis of range of motion (ROM), muscle strength test by the Medical Research Council (MRC) scale, analysis of postural changes, tests of the Berg Balance Scale (BERG) and Timed up and Go (TUG). Ten sessions were held per week with a duration of 45 minutes each. Among the proposed procedures are segmental and multi-joint resistance exercises, postural change training, static and dynamic balance, gait training, and carrying of objects. Results: The patient no longer presented with ankle edema; there was an improvement in muscle strength according to the Ashworth scale, graded ¾ globally, and a qualitative improvement in performing postural transitions. There was an improvement in the Berg Balance Scale (BBS) scores, from 15/56 in the initial assessment to 31/56 in the final assessment, and a reduction in the Timed Up and Go (TUG) test time, from 00:38.03 seconds in the initial assessment to 00:25.20 seconds in the final assessment. Additionally, there was an adjustment of the assistive device according to motor gains and gait safety. Conclusion: The physiotherapeutic intervention in an individual with tetraparesis more pronounced on the right, with distal predominance due to EGPA in an intensive rehabilitation program proved to be effective.
