Planning hospital discharge for people with disabilities or reduced mobility: good practices, challenges and improvements identified by the professional teams involved

Abstract

Objective: To analyze how health and support professionals working in the hospital network prepare for the discharge of people with disabilities or reduced mobility. Methods: An exploratory, cross-sectional study using an online questionnaire and non-probabilistic sampling was conducted between April and June 2022. Sixty-two professionals from teams working in public and private hospitals in Brazil participated. Data analysis was carried out using descriptive statistics, with the aid of the Survey Monkey® platform and "R" software version 3.3.0. Results: Among the 62 participants, 22.5% (n= 14) were male, 77.4% (n= 48) were female; 8.0% (n= 5) were physicians; 53.2% (n= 33) were nurses; 24.1% (n= 15) were physiotherapists; 9.6% (n= 6) were social workers; 1.6% (n= 1) were architects; and 4.8% (n= 3) were from other professions. 43.6% (n= 27) participate in preparing these individuals for discharge, 56.4% (n= 35) do not; 33.8% (n= 21) conduct pre-discharge home visits, 66.1% (n= 41) do not; 53.2% (n= 33) question whether there is provision for a home caregiver, 46.8% (n= 29) do not; 61.2% (n= 38) suggest improvements in discharge preparation, 38.7% (n= 24) did not respond. Conclusion: The findings show that, despite the participants' good practices, challenges and gaps persist regarding the need for improvements in preparing these individuals for discharge. Most are not involved in their planning, do not attribute relevance to home visits for assessing accessibility, training, and predicting caregivers, nor do they have knowledge about referrals to specialized services and community support. Individualized discharge planning is essential for continuity of care and performance of daily activities, considering functional limitations and dependence on family help in the short, medium, and long term.
