Estudo ergométrico evolutivo de portadoras de fibromialgia primária em programa de treinamento cardiovascular supervisionado

Fibromialgia é uma síndrome dolorosa crônica caracterizada por dor musculoesquelética generalizada1,2,3,4,5. Na última década, o exercício físico tornou-se promissor como opção terapêutica da síndrome6,7,8,9,10,11,12,13,14. Objetivo: avaliação ergométrica prospectiva de portadoras de fibromialgia primária (FP) em programa de treinamento cardiovascular supervisionado (TCS). Treze mulheres, média de idade de 48,9 anos, portadoras de FP, submeteram-se a teste ergométrico (TE) em esteira rolante, protocolo de Ellestad, no tempo zero, 3o e 6o meses de TCS. Os critérios de interrupção do TE foram cansaço e dor. Para o TCS, foi estabelecida uma faixa de 60% a 70% da freqüência cardíaca (FC) máxima, calculada pelo método de Karvonen. A assiduidade foi superior a 80% de 72 sessões, 3 vezes por semana, com duração de 60 minutos. Realizada a avaliação subjetiva da dor muscular e analisadas as variáveis do TE. Análise estatística: variância dos postos de Friedman e teste de comparações múltiplas15. Resultados: no 3o mês, houve aumento significativo da resposta cronotrópica. No 3o e 6o meses, foram significativos: aumento do tempo de exercício, capacidade funcional, trabalho total e diminuição da FC carga máxima comum. Não houve diferença significante da DPAS, duplo produto (DP), DP carga máxima comum e %FC máxima. Comparadas com o final do TE do tempo zero, a maior porcentagem de pacientes atingiu cargas mais elevadas e a mesma intensidade de dor no 3o e 6o meses de TCS. Conclusão: a partir do 3o mês de TCS, as portadoras de FP apresentaram maior tolerância à dor muscular e ao esforço, melhora da capacidade funcional cardiovascular e muscular periférica.
