Sialorrhea is a frequent comorbidity in children with cerebral palsy, with significant negative impact on personal, social, and educational domains, as well as on the quality of life of patients and their families. Objective: The aim of this clinical update was to review the literature and describe recent advances in the treatment of sialorrhea in children with cerebral palsy, highlighting available interventions and their effectiveness in clinical practice. Results: Evidence shows that management options include speech therapy, pharmacological treatments, application of botulinum toxin, behavioral strategies, and, in refractory cases, surgical procedures. Conclusion: Individualized and progressive management, preferably guided by a multidisciplinary team, is essential to reduce complications and optimize outcomes. However, there is still no consensus on the most effective approach, and gaps remain in the literature, particularly regarding long-term efficacy and comparative studies.
